I studied TOC under Eli Goldratt. He spoke of how in the Israeli military, after they hand out a medal of honor, they immediately find out what happened that required such a feat of bravery and fix it. They don't want to continually face that type of problem or situation. That too shows leadership and how preventing things from going wrong is a sign of smart leaders.
